Appointment Cancellations
Appointments may be canceled or rescheduled prior to the scheduled time.
IHS reserves the right to assess a late cancellation or no-show fee of up to $25 at the Provider's discretion.
Repeated missed appointments may result in discharge from the practice.
Late Arrivals
Late arrival may require shortening or rescheduling of the appointment.
A fee of up to $25 may apply.
Refund Policy
Completed clinical services are non-refundable.
Medication and laboratory charges follow third-party pharmacy and laboratory policies.
